Student’s Contract

Within this agreement the term Student(s) refers to Students, parents of students, or other authorized responsible parties, and the term Teacher refers to teachers, administrators, management or any staff member of Marina Sultanova’s Music Studio, LLC. Students are to sign and hand in a student contract with their first payment of lessons to the teacher. This student contract ensures that the students understand this business arrangement.
Student’s Responsibilities
Materials – Students are responsible for the purchase of their own books, instruments, and other materials needed for the lessons they are receiving. We will refer our students to stores and websites where they can obtain these items, however, the teacher is not responsible for providing such items personally and at their own expense. Copies of the books can be done in the studio for $0.50 per page.

Practicing – The teacher reasonably determines how much practice is necessary and recommended for each student and will encourage students in their practicing. Once a lesson is over, it is the students’ responsibility to practice and complete homework. Progress will be made most quickly by those students who maintain the recommended practicing time. Parents are also encouraged to be involved in their children’s practice. Taking time to listen to the children play their weekly songs or creating a set time for practicing in the students’ daily routine are good ways to become involved. Teachers do not give any guarantee that a particular progress will be reached within a number of lessons.

Attendance – Students are responsible for their time slot with Sultanova Studio. If a student is not available for their lesson for any reason (including illness or other required school function), it is the student’s responsibility to notify the teacher at least 12 hours in advance. If the missed lesson is due to an illness or other required school function and the student notified the teacher at least 12 hours in advance, there will be no charge to the student (only 1 lesson per month). If the missed lesson is due to any other reason, or if the student neglected to give the teacher the required notice, the lesson will be charged as provided in the table of lessons, times, and charges for the lessons above. The teacher, on their own initiative, may reschedule the lesson, but is not required to do so.

Payment is due before the lesson starts. Checks are to be drawn to Marina Sultanova’s Music Studio, LLC and presented to the teacher before they sit down with the student for their lesson. In the event the provided check bounces, a $25 fee applies to the original payment. The studio does not provide any credit; therefore, the lesson is to be cancelled, if the payment is not presented in a timely fashion. The payment for packages expires within 3 months from the date of the purchase. Once the payment is processed, it’s non-refundable.

Monthly Subscriptions – By purchasing a monthly subscription the student agrees to an initial and recurring monthly payment at then-current monthly-subscription rate, and accepts responsibility for all recurring charges until you cancel your subscription at any time according to the Cancellation Policy below. If for any reason we do not receive payment from the student’s credit card provider or if the credit card expires or rejected, the student agrees to pay all amounts due upon request by providing a valid credit card before continuing with the lessons. The student authorizes us to charge the outstanding fees against any credit card the student has on file with the studio. We reserve the right to take all steps necessary to collect amounts due from you.

Cancellation Policy – Students may cancel the subscription at any time before the charge for the new month goes through. Once the payment has been applied to the credit card on file, the fee is non-refundable.
Assumption of risk and release from liability – Parents recognize that there are certain inherent risks associated with the above-described activity and assume full responsibility for personal injury to their children and the third persons involved, and further release and discharge the teachers and staff of Marina Sultanova’s Music Studio, LLC for any injury, loss or damage of property arising from such activity.

Photo/Video Release Permission
As a (parent or guardian of this) student, I hereby consent to the use of photographs/video taken during the music lessons at Sultanova Studio for publicity, promotional and/or educational purposes (including presentation or broadcast via internet or other social media sources). I do this with full knowledge and consent and waive all claims for compensation for use, or for damages.
I hereby release and agree to hold Marina Sultanova’s Music Studio, LLC harmless from, and waive on behalf of myself, my heirs, and any personal representatives any and all causes of action, claims, demands, damages, costs, expenses and compensation for damage or loss to myself and/or property that may be caused by any act, or failure to act of the studio, or that may otherwise arise in any way in connection with any services received from Marina Sultanova’s Music Studio, LLC. I understand that this release discharges Marina Sultanova’s Music Studio, LLC from any liability or claim that I, my heirs, or any personal representatives may have against the studio with respect to any bodily injury, illness, death, medical treatment, or property damage that may arise from, or in connection to, any services received from Marina Sultanova’s Music Studio, LLC. This liability waiver and release extends to the studio together with all owners, partners, and employees.
